Once you equip a fishing pole, how do you use it, I right clicked I hit the hotkey, but I dont know how to cast it out.

Go to your general skill tab, and there is a fishing skill. Drag that to an open spot on a bar, and once you have your pole equipped, go to some water and just click.

To use bait right click on the bait in your inventory and then your cursor will glow, then left click on your fishing rod (it can be equipped or in your inventory - doesn't matter).

Once you're fishing skill gets high enough you'll be able to start fishing in the little whirlpools you see along the coastlines and in rivers and lakes - those pools let you catch special fish that can be used in alchemy or cooked into better food using the cooking skill.

If you see a small pile of boxes and barrels floating in the water instead, by fishing in that you'll get boxes that you can open with stuff inside - usually cloth of some type, occassionally ore or metal bars and once in a great while a green item (as well as some pocket change).

As has been said put the icon in a bar slot and click on it , hold mouse over the float and right click when it dips into the water , you will see when you hold the mouse in the right place as you get a "gear cog icon "

At first you will need bait up to about skill level 50 stay by the trainer in a low level area at first and get above 50 to get the next level training off the trainer ( skill 50 minimum ) for me i find it far easier to stay near the trainer and get skill up to at least 95 as that means i can fish slightly higher level waters like in the barrens without having a lure , it takes quite a while to get used to fishing but is well worth it as there is some very good selling fish like Deviate fish and boxes fished from the rubbish pools you will find.

Once the cast is in the water and you have that gear icon, you right-click on the gear once it bobs up and down (the indication that you've hooked a fish). This is accompanied by a minor splashing sound that can be tough to distinguish at first from the general water sound, but you'll get the hang of it. Thats how you reel in your catch. It'll say "no fish are hooked" if you click too early or late.
