Prior to getting to Outlands officially, I was ported to Shattrath where I rasied my Alchemy above 300. I was also walked to Honor Hold where I raised my Herbalism past 300. After that, and again before officially entering Outlands, I made a good 50-60 (outland mat) potions and elixirs and did not have one discovery proc. Now that I am officially in Outlands, I have made well over 100 (outland mat) potions and elixirs and still no discovery proc, and this is after the patch.
Am I doing something wrong? Did I break the cycle by going to Outland early? Do I have to buy my imbued vials in Outlands too?

The patch made it more likely to occur.
Prior to the patch, we had 2 alchemists in guild with 1 discovery each.
Since the patch, every guild alchemist (except 1 poor unlucky one) has had at least 1 discovery. Some of them are up to 3 discoveries total. I myself finally got lucky and discovered the Flask of Mighty Restoration.

I've got 3 discoveries post 2.1. All of them are kind of useless but it's better than nothing!

I discovered Transmute Primal Mana to Primal Fire last night.

I made at least 300 pots post patch before i got one.
I am sure you did nothing wrong, just that even with the rate increased, it is dang low.

I'm curious, do you have to be 375 alch before you can get a discovery? I guess the only way to know is if someone has found otherwise. Thanks.
The only requirement for a discovery is to make a potion, elixir, or transmute with an Outland ingredients i.e. Felweed or Primals.
150 times is not a significant number for discoveries, although you should probably have got one in this time span.

They seem to be totally random, for the moment I've discovered Flask of Shadow Prot, Flask of Mighty Restoration, Super Rejuv Potion and Transmute Mana to Fire. I've discovered the flasks while doing pots, the potion while doing an elixir, and the transmute while doing a transmute, so I don't think there's any correlation between what you're doing and what you're discovering.

Was getting frustrated, but after about a month with nothing I received a Transmute primal life to Earth when I did a primal might transmute.
Extremely happy because it is fairly easy to come by the Primal Lifes, but serious grind to do the Earths.

Last night, I got the opposite, primal earth to life.
