I'm at 225 first aid, they doctor will not give me the quest. The stick says for the production skills you need to be lvl 35, but does not list first aid. I'm lvl 23. Is this the requirement for first aid also, 35, or is it different?

The level requirement, as with every skill in the game, is 35 to get above 225. It used to be 40, but they bumped it back to align with primary skills.
