Ivy on the phone was a definite "Seriously?" Moment. Maybe Barbara thinks Jim is into that sort of thing now?
As for the nurse bit, I did assume that she was not a "current" patient. They did say she was from before they shut the place down and had apparently been living in the basement, forgotten. I just figured she showed up at the reopening and got a job because they suck at staffing.

I rewatched the episode. It's clear that she wasn't a current patient. She was a past patient, who happened to get left behind in the previous closing, and infiltrated the staff during the reopening (not hiding, but instead actually being hired as a nurse). The "ah ha" moment for the head honcho was when he realized that she was the only one that was not a coworker of his from the past that he knew.
