A lot of folks seem to. I've brought the meaning up around a lot of folks when it comes on (on my playlists, or on rock stations where it's still rather frequent). I've probably asked about 7-8 people, and I've yet to run into one who had an inkling of the actual meaning.
I think that most people only hear very defined vocals, particularly in the chorus, and the most easily identifiable lyrics in this one are "I will treat you well, my sweet angel, so help me Jesus."

I made a fairly concerted effort to uncover the meaning once a few years back and pieced it together from a few band interviews, some anecdotes, the menacing way it's sung, and my interpretation of the pretty straightforward lyrics.

I don't think it's necessarily referring to any specific case (some say that it's about a particular string of teen girl disappearances), but the kidnapping, rape, and murder all seem to be pretty implicit.
