It appears that if you fill out the form, they'll show you where to go vote and what a sample ballot looks like. Don't worry though, there will be instructions when you get there. If you don't have electronic voting machines (which seems unlikely) you'll probably either have the lever kind, the kind where you punch out your selection with a small metal thinger (bewarrrrrrrrrrre the hanging chad!) or you'll have to fill out an arrow to select your candidate/option. We have the kind where there's an arrow with a section missing that you fill out in sharpie or whatever to complete the arrow...
