It's a cultural thing, really. It does have its origins in the whole "wipe with one hand, eat with the other" concept, but I'm reasonbly sure that toilet paper is
mostly available in the Middle East, and certainly would be to anyone in a position to shake the US President's hand.
There are some areas where it's still important though. My former roomate went on a dig in Jordan a few years ago. The entire camp ended up with dissentary (her description of the busride to the hospital in Amman is a real appetite breaker), and she still carries with her some bugs in her intestines that will forever affect her digestive system. All as a result of someone not following proper water usage methods. We tend to take for granted that we'll have clean running water wherever we go, so we don't worry too much about things like which hand we use for what. In this case they did have toilet paper, but no running water. In a situation like that *anything* you get on your hands will fester in the water so you have to be constantly aware of where you put your hands and what you touch.
It's somewhat irrelevant outside of that situation except as an insult.
