Break the routine, as was mentioned. Unles you're one of those sad fuc
kers that need to chew nicotene gum or wear a patch, it's just a matter of how badly you want it. Also, don't drink for a few weeks, until you've got a lock on not smoking. Nothing screws up your resolve like a few pints.
