Now thats a toughy, I'd have to say... Kick in the groin. That's over in 5-10 mins unlike the eye-poking which can bug you for hours.
It really depends on the desired effect.

I was the foreman for a jury presiding over a murder trial where a woman used a TV cable to whip her 6 month old fostor child. She also swung the baby around by her heels and cracked her head into the doorjamb.

getting poked in the eye definitly wont "hurt" as much. the eye doesnt have a whole lot of feeling to it, although it can easily get irritated.
the jewels though? at times it seems like their only purpose is to create as much pain as possible.
