Random note, just about the ODing part. I work for an addictions education center, and a lot of our trainings are on heroin right now (since it's the biggest issue in NJ atm).
Most heroin addicts start with prescription pills. But pills are so expensive, that their habits quickly end up costing $1-5k a month. Once all their lines of funding dry up for pills, they move to heroin (which is one of the cheapest drugs you can buy).
And at that point, it's not really a choice. Addiction, particularly when you're in a state of withdrawal, essentially turns off other high-order desires in your brain. Sometime starting to go through withdrawals (or already in them) because they can't get pills IS going to take heroin if they have access to it. It's essentially a survival behavior at that point - their brain is screaming at them that they need it to survive, and nothing else matters next to that.
And because of that particular issue, even knowing rationally that a dosage could kill you ceases to matter, because depending on how dependent you are, and how deep into the withdrawal you've gotten, your brain just isn't capable of rationally processing that information. The only thing that matters is that your instinct is that you need the drug to survive (even if you know it'll kill you).
Also worth noting that, being so dirt cheap, heroin is usually one of the (for lack of a better word) filthiest drugs out there. It's cut with all kinds of crap. We had a ton of deaths in NJ this year from tainted heroin coming out of Atlantic City.
Really quite sad. One of the worst parts of heroin abuse is actually the sheer number who get started on this path of addiction literally. Addictions to prescription drugs, legitimately prescribed for pain, is extremely common. Most people manage to keep their addiction reasonable. Some of them don't.
Obviously, the less wealthy you are, the more likely you'll end up on heroin. A lot depends on your ability to pay for the pills.
