I have it running 24/7 and even when I click on items that are not in the database, or items that need to be updated, it never requests or sends the data. It sees the item and decides to not request it. Any idea what's up?

In order for it to pick up updates (for example, after the EQ servers go down for a patch), you must quit out of Item Collector and restart it. It's a good idea to do this periodically in general in case any other bugs have been fixed.
