Gamepad not working in FFXIFollow

Here's the situation. I have a PS2 gamepad that I'm connecting to my computer through a USB adapter (a SmartJoy PLUS, to be specific). It works perfectly. Windows recognizes it, every other game I've used it with works fine, and I know that FFXI works with it, too, because when I last played FFXI several years ago, I used it all the time. I just recently decided to reinstall and reactivate FFXI, however, and now it's not recognizing my gamepad. I can set it up in the FFXI Config utility and the tester there works fine; I can also set PlayOnline to use it, and it works fine there, too. When I actually go into FFXI, however, it doesn't respond to the gamepad at all, but the game seems to otherwise function perfectly.

Here's the weird thing: it worked once. While I was messing around with settings trying to get it to work, there was one time that I start FFXI and it worked perfectly, so I thought I had fixed it. I played the game for a while and then quit, but the next time I started the game, it stopped working, and it hasn't worked again since then.

So.. any ideas?

Did you enable the gamepad to be used in the Final Fantasy XI Config application?

If you didn't do this then the gamepad won't work.
