1. At the start of the instance say in /p "Let me know if you want CC. A purple diamond would be great =)" Communication at the start of the instance can save a lot of confusion and repair bills.
2. If the tank is of the rush in and tank everything variety, skip to last step.
3. If the tank marks a target for you, hit your CC of choice immediately on the pull (unless asked to CC before the pull, AKA a "CC-pull"). Most decent tanks will back-peddle their pulls away from where the mobs started. This keeps them a short distance from the chaos that is melee.

Keep the target on your screen. Watch it like a hawk. Your priority is to keep that mob locked down. DPS is secondary. There are also addons that can track your CC timers. I don't use them currently so I'm not sure what to recommend. Check wowinterface.com or wow.curse.com for options.
5. If your CC target breaks, re-cast immediately. Do not finish your Shadow Bolt.
6. If your CC target breaks due to tank AOE try
Death Coil (which forces it to run around a bit, not effected by Fear glyph) and time your CC cast to land when the mob is away from the tank. Failing the Coil, you can also spam
Searing Pain or
Fel Flame (while moving away from the group). This, hopefully, will force the target to focus you and move it out of the tank's melee range. Use line of sight for casters. Pray the mob isn't dotted.
7. Once the tank purposefully breaks the CC, skip to the last step.
8. Use either a mouseover or focus macro. Having to click a mob to select it adds up to 1 sec. to your cast time. Time you don't have to waste when a target breaks and is eating your tanks face. I have used both and currently prefer this mouseover (helps to have enemy nameplates showing!):

This is my old focus macro (which I still use in some circumstances). You need one macro for each type of CC with this one. Though, looking at it, might be able to combine with modifiers.

10. When all above are done, PEWPEW KILL IT WITH FIRE!!!!! ZOMG RAWR!!!!!!
It's possible to keep 3 mobs CC'd with practice (hello Stonecore/Bastion trash!). Not all tanks will want CC but it's a vital skill for any warlock to have when it is called upon. Practice whenever you can!
