In PvP you leave combat when no action has been taken for 6 seconds. If you spec into Imp Gouge, this gives you a 5.5 second gouge, which should allow you to restealth just as your opponent comes out if you hammer on your hotkey.

In PvE the only way to leave combat is to kill your target or not fight and kite it far enough where it loses interest.

Edit: Fixed durations, thank you Theo.
Edit#2: Fixed again, L2wowhead. Thanks Kavekk

Or vanishing, but that kind of ruins the point.

On the other hand, if you're about to die, don't bother running from a mob to get out of combat so you can restealth; just vanish.

It's 6 seconds, but yes.

On a related note, in PvP, "manual restealthing" is often used while your opponent is blinded, so that you can sap them and wait for the twenty-second cooldown on kidney shot to finish up. Restealthing is key to prolonging a stunlock.

Gouge is 5.5 seconds talented, not 6.5. That's why imp gouge to restealth is very dodgy.
