are DKs only able to get in there starting area or is everyone able to go there?
Um I would assume anyone can go to the DK starting area since it is considered neutral (pve) or contested (pvp). The hold itself is a safe zone.
Only DK.
And once you do the final quests, you cannot go back (They'd be nothing left to do anyway).

what about changing runes? Doesn't the DK's have to have access to the hold in order to re-forge their weapons?
You have access to a runeforge after you become 'free.' Just open a deathgate and there's a runeforge there.
Ebon Hold is a 'city' in its own right.
Before you complete the 'DK experience', you're essentially in an instance, that include ebon hold and the a small part of the world below, where most of your quest happens. After the the 'DK experience', you then have access to the real Ebon Hold and the flight path leaving out now go to location in Azeroth, rather than the 'DK experience' zone.

and, just to clarify, only DK's can go to the real Ebon hold?

From what I understand so far (from what I've read), yes, only DK's have access to the Ebon Hold flight paths and the Deathgate spell. I guess other classes can't get there.
