1. 70 after all your quests are completed
2. Scryer dudes in SMV are pretty decent
3. Try to keep blue weapons within 5 levels of your current level if possible.
4. check out WoWwiki.com for reputations.

I would grind from lvl 68 to 70. Then finish all the quests you can pick up in BEM, NS, and SMV for easy gold.
grind instances in outlands whenever possible to give your future rep grinds a head start.

Okey, don't know why I assumed that there is another interval of levels that I should grind before Outland.
Amen to grinding instances. I had planned to do that with Firecrotch but I never really got round to it.
Level 69 is an easy grind at Isle of Quel'Dammitiforgotwhatitscalled. The humanoid mobs that are used for one of the quests spawn really quickly and are a pretty easy take-down.

Another thing to consider is professions. If you're going to have to grind mats for that anyway, might as well grind it while you still get xp. Don't quest to 70 only to find that you have to head back to Nagrand to kill 200 clefthoofs for zippy xp.
edit: FWIW I was killing them last night for ~1250xp (rested) on my 63 drood skinner/leatherworker.
