You should really be using Muti to build up Combo Points. Shiv is more of a utility thing. I don't know too much about Mutilate, but I know Theo was rocking it for a while so to quote him:

You really shouldn't have to be spamming it for combo points. But I would say that if you find the spec fun then just go for an offhand. Maybe someone who is more up to date on the current effectiveness of pvp builds can give you a more informed insight into whether you should go for another weapon. To be honest I couldn't answer that question myself (*Points to sig*).
