Get totem timer from curse gaming. Its an add-on that puts a timer beneath your totem icon and it also organizes your totems into 4 pull-down buttons, 1 for each totem family. Just mouse over it and down to the totem you wish to cast. You can also bind them to hotkeys alot easier. For example, if #1 is the earth totem family, then 1 would be earthbind, <shift> 1 would be strength of earth and so on. Pretty handy.

Don't think so, but you could always scale it down very small and use key bindings for the totems.

I've found that people generally don't put on their toolbars:

Flametongue Totem
Windwall Totem
Nature Resistance Totem
Frost Resistance Totem
Fire Resistance Totem
Sentry Totem
and sometimes Stoneclaw Totem.

Others, however, make a macro button for the Resistance totems, making one of them left-click, another right-click, another scroll-mouse-button.
