/cast [target=mouseover,harm] Ice Lance; Ice Lance
That's the macro I use. Just hover your mouse over the totem, press the macro, and you'll shoot an Ice Lance at the totem. If your mouse isn't hovering over anything, you'll shoot an Ice Lance at your current target.
