For the book itself you need to do the solo instance (escape with Mazog) then do the three 3-man instances. You get one piece of a non-radiance set of three peices of armor form each of these. You do not need to kill the final boss in the first two instances to get credit for the chapter. You kill the final boss in the first two for a barter stone (and other goodies). You do need to defeat the boss in the last 3-man to get a third peice of armor from that set.

The radiance set of three pieces is acheived by doing the first two 3-man instances (Kill boss 4 times) to get 4 barter stones from each. You need 7 barter stones from the 6-man Hall of Crafting instance for the third piece of radiance gear(chest).

There are additional quests (starts with Haldir who sends you to Chamber of the Crossroad dwarf NPCs) for the instances.

Usually referred to as:

HoM: Hall of Mirrors, instance is located outside north of the Balrog corpse in Zirkzigil.

WW: Water Works, take the first bridge on the right past where the Deep Way comes in. Building across the causeway is where it starts.

HoC: Halls of Crafting, Door #3 next to FG and the Forges in Flaming Deeps.
