Family InfoFollow

Hello,
Would someone help me out with how to make Family in EQII, I have read that you can have a family and also be in a guild, but what are the requirements for making a family and where can I look to get the info. Thank you for your help in advance.

Galdric 15 Shaman going Defiler
Triinx 11 Crusader going Shadowknight
Daaku 11 Summoner going Necromancer
Nuwaadeeb 10 Predator going Assassin

To get a surname now, you use the /lastname (I think... haven't made it there yet) command to choose your surname at level 20.
