Heard some say yes and some say no but do the mana regen items stack? Example: If I have a mana regen +3 item will it stack w/ a mana regen +2 item making my mana regen +5? I am alittle confused by thsi b/c I thought that item abilities did not stack (such as haste)but I have had some say that flowing thought or mana regen if you will does.....

Yes it stacks up to +15 worn mana regen.
That is correct.
You may wear multiple Mana Regen Items/Augs up to a Max of 15 points. (Unless u use AA's to increase that number)
If you look at Akornn's (My Druid) Magelo, He has a +3 Mana regen shield Aug'd with a +2 Mana regen stone. So when you look at the item's properties (eiter in Magelo or ingame) it shows as Mana Regen +5.
The Same applys to Health regen but I believe the Max may be greater..not sure though.

then you can also get arch type aa called Mental Clarity to bring you to FT/Mana Regen 18 (no aditional FT/Regen items needed, this is FT you keep nakid even, +3).
then you can also get Omens lvl AA's - Expansive Mind, it is 5 lvls that alow more of your "stackable" FT/Regen to "count" thus brining your total "stackable" regen to +20 (need items for this) and OVERALL regen to +23.

Now, do all THOSE regen points stack along with all CAST points (KEI and the like) as well or is there a trade off or cap involved there?

Yes.
My only quible is the word "all".. I absolutely hate absolute statements. Not all the spells stack. The ones that do, totally stack with worn mana regen (and hp regen).
As do bard songs (*drool* cool.. clear.. mana)

KEI/Clarvoyance (enchanter)

but some items have "click" mana regen buffs (mpg trial sholders, VT click sholders ect..), and most of those DON'T stack with each other, but DO stack with KEI/SA/OAK

Necro Lich spells do not stack with (and are generally superior to) KEI buffs, but will stack with bard, druid, clicky, etc. mana regen.
