I use 3rd person, completely scrolled back, almost exclusively. It helps me keep track of where the mobs are and where my groupmates are, as well as making it easier to judge distances in relation to my self. (It also makes it easier to see if an add has broken loose, or who's getting beat on at a glance.)
If there are a lot of bodies, or if lag is high, I'll occasionally switch to 1st person, as it seems to help with both targeting and lag.
Normally, I'm using 1600 x 1200 on my main. But occasionally I'll bump it down a notch if I've got eye strain.
I used to have problems targeting the mob instead of my own toon, but very quickly I learned how to look "past" mobs instead of "at" them, so as to avoid that problem.
LDoNs end up making me use 1st person more than I'd like, because of the enclosed environments.
