Have a look at this it is pretty self explainitory.
I made a Plane of Power progression Flow Chart.
You should be able to see what needs to be done a little better.
You can get into Tier 2 without having the Flags if you're Level 55 and above.

This link is useful too, and refers to the clues that the Seer in PoK gives you to what flags you have and need.
